What to bring to appointments:

Disclaimer of Payment and Insurance:

  • Copays are due in full at the time of services. This include all coinsurance and deductibles.

  • We accept all major credit cards, cash as to be exact because we do not carry change. We do accept checks!

  • Payment plans can be arrange through the billing department.

  • If the patient is a minor (18 years or younger), the parent or guardian is responsible for payment of the account, in accordance with the policies outlined above.

  • If your account is overdue longer than 90 days, it will be referred to a collection agency. This is a last resort, done reluctantly and after we have exhausted efforts for voluntary payment.

Missed Appointments and Prescription Refills

  • Appointments must be canceled at least 24 hours prior to the scheduled appointment time. Failure to show up or cancel your scheduled appointment will result in a charge of $95-$150 or higher fee without proper notice.

  • All appointments must be confirmed by phone at 4 PM the day before your appointment, or it will be canceled!!! No exceptions!!!

  • Medication refills will not be processed for patients who fail to keep their scheduled appointments.

  • Medication will not be refilled after hours or on weekends. Refill request(s) will be processed within 48 hours of receiving the request. There will be no early refills on medications.

  • For Patients who are prescribed narcotic medications: Failure to take your medication as prescribed or to obtain duplicate prescriptions from other physicians may result in dismissal from our practice.

  • Non-emergent messages left for the physician will be returned within 48 hours. To receive a response from the physician, you must be prepared to leave a detailed message/reason for your call. The physician may return your call personally; however, in some cases, he will instruct his staff to return your call.

  • If you feel your situation is urgent, please report to the emergency room. (i.e. stroke symptoms, uncontrolled seizure activity, chest pain). Do not leave a message and wait for a callback.
